REQUIRE­MENTS FOR NEW CONNECTION

1. Appli­cants must attend the Pre-​Membership Edu­ca­tion Sem­i­nar at the DASURECO Offices on these schedules:

• Main Office – Wed & Sat (8:30 AM)

• Malalag & MaBaMa – Wed (8:30 AM)

• Malita – Tue (8:30 AM)

• Padada – Thu (8:30 AM)

• JAS – Wed (8:30 AM)

• GMall Office – Mon­day to Sat­ur­day (will start at 1:00 PM)

2. From the Local Gov­ern­ment Unit

(Art. 6 of the Magna Carta for Res­i­den­tial Elec­tric­ity Consumers)

• Orig­i­nal Elec­tri­cal Plan (signed/​sealed by a PEE)

• Orig­i­nal Cer­tifi­cate of Final Elec­tri­cal Inspection/​Completion/​Occupancy (from The City/​Municipal Engi­neers Office)

• Orig­i­nal Approved Elec­tri­cal Per­mit (from The City/​Municipal Engi­neers Office)

• Barangay Cer­ti­fi­ca­tion attest­ing that the Barangay Cap­tain knows the appli­cant to be a res­i­dent and owner of the house and/​or lot being applied for elec­tric service.

3. Pho­to­copy (Bring Orig­i­nal) of Proof of Lot Own­er­ship i.e., Cer­tifi­cate of Title (Best Evi­dence), Deed of Sale recorded in the Reg­istry of Deeds, Pag-​Ibig cer­tifi­cates, etc. Tax Dec­la­ra­tion may be con­sid­ered but will not give holder as much pro­tec­tion as one with a bet­ter right over the lot.

• If appli­cant owns the build­ing but not the land, a Nota­rized Affi­davit of Under­tak­ing (U1) to be exe­cuted by the landowner under­tak­ing to be jointly and sev­er­ally liable up to two (2) months for the unpaid elec­tric­ity bills of occu­pant in case the latter’s bill deposit is not suf­fi­cient to pay for his/​her arrearages;

• If suc­ces­sor, Proof of suc­ces­sion to the property;

• If ten­ant of privately-​owned premises/​government-​owned prop­er­ties, Nota­rized Under­tak­ing (U2) exe­cuted by landowner/​LGU and tenant

4. Final Inspec­tion Report (From the Insti­tute of Inte­grated Elec­tri­cal Engi­neers or IIEE)

5. Pay­ment of Bill Deposit (one month esti­mated billing), Mem­ber­ship Fee of P5.00, and excess in ser­vice drop wire (if any)

6. Final Pro­cess­ing and Pay­ment of Bill Deposit (one month esti­mated billing), Mem­ber­ship Fee of P5.00, and excess in ser­vice drop wire (if any) will be at the lower ground floor, Gaisano Mall Satel­lite Office, Brgy. Tres de Mayo, Digos City.

BASIC REQUIRE­MENTS FOR NEW APPLICANTS:

1. Atten­dance of Pre-​Membership Seminar

   • Main Office, Cogon, Digos City — Wed & Sat 9 AM

   • Malalag & Matanao — Wed 8:30 AM

   • Malita — Tue 8:30 AM;

   • Padada — Thu 8:30 AM

2. 1 pc. 1 x 1 id pic­ture of the appli­cant (for joint mem­ber­ship 1×1 id pic­ture for husband/​wife).
3. Elec­tri­cal per­mit from Local Gov­ern­ment Units.
4. Cer­tifi­cate of Final Completion.
5. Cer­ti­fi­ca­tion of Own­er­ship (Deed of Sale/​Transfer of Rights/​Extra-​Judicial Partition).
6. Elec­tri­cal Plan.
7. Final Inspec­tion Report (IIEE inspectors).
8. Final pro­cess­ing at ISD (Dasureco Main Office).

BASIC REQUIRE­MENTS FOR NEW APPLICANTS:

1. Certification/​Resolution

   • If in Barangay — Barangay Resolution.

   • If in Munic­i­pal­ity — Cer­ti­fi­ca­tion from Municipal’s Mayor and Municipal’s Treasurer.

2. Mate­ri­als must be pro­vided by the applicant.
